FEES and INSURANCE

CRSA physicians share your concern about the increasing cost of medical care. Charges for office visits are based on the time required, the complexity of the problem, and if special tests are needed. For this reason, charges may vary from visit to visit.

We request that office charges be paid at the time of service, unless we are billing insurance for you. Your insurance policy is a contract between you and your insurance company. Payment is the responsibility of the patient, even when insurance coverage is available. We will be happy to submit your insurance forms for you and offer guidance in any way we can.

Medicare & Blue Cross/Blue Shield of Maryland:
We are participating providers of both these insurance companies. We will submit claims to them, and they will pay us directly.

HMO Plans:
We are contracted with various HMO's and PPO's. It is your responsibility to obtain a referral and/or authorization to be brought with you for your visit to our office.

Supplemental Insurance Policies:
If your supplemental insurance is a company with which we have a contract, we will submit a claim for the additional payment. If we are not contracted with your supplemental insurance company, it will be your responsibility to satisfy the remainder of the claim and seek reimbursement from that company.

Deductibles and Co-Payments:
These applicable fees are expected to be paid at the time of service, regardless of your insurance coverage.

Surgery:
We make every effort to contact your insurance company before surgery takes place to let you know what your portion of the surgery cost will be.
